(MS Word) RECOMMEND:
1) you amend the consent paragraph to specify your Branch name
2) paste the text onto your Branch letterhead
3) you have a Photograph Policy, or at least understand its intent,
to ensure you are in accordance with Canada's
Privacy Act. (For example: National
Office Photograph Policy)
